Your body did not fail to bounce back. The schedule did.
Most American women are back at work twelve weeks after giving birth, and for many of them that leave is unpaid. Twelve weeks is shorter than the recovery timeline for the tissue involved. Abdominal separation is still present in roughly a third of women a full year after birth. The deadline was set by employment law, not by biology.
There is a version of this conversation where someone tells you to be patient with yourself, and it is well meant, and it changes nothing. This is the other version.
What twelve weeks actually is
The Family and Medical Leave Act of 1993 guarantees eligible American workers twelve weeks of unpaid, job-protected leave. There is no federal paid maternity leave. Eligibility is narrower than people assume: your employer needs at least 50 employees within 75 miles, and you need 1,250 hours worked in the previous twelve months. A large share of working women do not qualify at all.
So twelve weeks is not a medical recommendation about when a body has healed. It is the outcome of a legislative negotiation in 1993.
What the tissue is doing in the same period
Abdominal separation. In a Norwegian cohort followed by Sperstad and colleagues and published in the British Journal of Sports Medicine in 2016, diastasis recti was present in 60% of women at six weeks postpartum, 45% at six months, and 32% at twelve months. A third of women still had measurable separation a year out. At twelve weeks, most do.
Connective tissue. Relaxin rises through pregnancy and joint laxity does not resolve the moment the baby arrives. Ligaments and tendons remodel over months, not weeks, and they remodel in response to load rather than rest.
Blood volume and cardiac output take roughly six to eight weeks to return to baseline, which is part of why the tiredness at week ten is not the same thing as being unfit.
None of that is a reason to do nothing. It is a reason to stop measuring yourself against a twelve-week line that was never about your body.
The part that actually moves
This is free and it is most of the result.
Sleep, before anything else. Short sleep changes the composition of weight lost, shifting it away from fat and toward lean tissue, and it raises appetite signalling the next day. With a newborn you cannot fix this completely. You can stop treating it as the least important variable, because it is the most important one.
Protein, earlier in the day. Most people under-eat it at breakfast and cannot catch up by dinner. Moving protein forward matters more than raising the total.
Load the tissue, do not just rest it. Resistance work two or three times a week is what protects muscle during any deficit, and what tells connective tissue to remodel. Start far lighter than your pre-pregnancy self and progress on a schedule rather than by feel.
Walk. Unglamorous and the single most consistently underrated variable in this entire literature.
Get the abdominal wall assessed rather than guessing. A pelvic floor physiotherapist can tell you in ten minutes what you are actually working with, and the exercises that help a 3cm separation are not the exercises that help a 1cm one.
What the research on tissue repair compounds actually says
You will see BPC-157 and TB-500 discussed in recovery contexts, so here is the honest state of it.
BPC-157 is a synthetic peptide derived from a sequence found in gastric juice. The published work is overwhelmingly animal research, largely rat models, looking at tendon-to-bone healing, ligament injury and gut mucosal damage. Several of those studies report faster healing against controls. Human clinical trial data is very limited, and the compound is not approved for human use anywhere.
TB-500 is a synthetic fragment of thymosin beta-4, studied mostly in animal models of wound healing and cardiac injury for its effects on cell migration and new blood vessel formation. Again, animal data, not human approval.
What that means in practice: these are laboratory research compounds with interesting preclinical results and no established human safety or efficacy profile for postpartum recovery. Nothing above is a recommendation, and anyone telling you they know how these behave in a postpartum body is telling you something the research does not support.

How long does it actually take to recover from childbirth?
Longer than twelve weeks for most tissue. Abdominal separation was still present in about a third of women at twelve months in a 2016 cohort study, and connective tissue remodels over months.
Is diastasis recti permanent?
Not usually. Prevalence fell from 60% at six weeks to 32% at twelve months in the same cohort, which means most resolve, though a meaningful minority do not without targeted work.
Does the United States have paid maternity leave?
There is no federal paid maternity leave. FMLA provides twelve weeks of unpaid, job-protected leave to eligible employees only, and eligibility rules exclude a large share of working women.
Should I do core work postpartum?
Yes, but the right kind. Get the separation measured first, because the appropriate progression differs substantially by how much there is.
Why am I exhausted even when the baby sleeps?
Blood volume and cardiac output take roughly six to eight weeks to normalise, and sleep fragmentation affects recovery independently of total hours.
